Who is eligible?
To be eligible to select the UMP Plus – UW Medicine Accountable Care Network, you must be SEBB benefits-eligible and live in Benton, Franklin, King, Pierce, Skagit, Snohomish, Spokane, or Thurston counties, and are one of the following: employees and their enrolled dependents, SEBB continuation coverage (unpaid leave) members and their enrolled dependents, or SEBB continuation coverage (COBRA) members and their enrolled dependents (not enrolled in Medicare Part A or B).
